Does your DSA membership card show the wrong name? Please fill out this form with the email address currently associated with your DSA membership and update your DSA membership to reflect the name you go by!
We will include a new membership card for you in the upcoming batch of cards and you should see it in the mail in 6-8 weeks (pending any delays due to COVID-19.) If your membership is expired, we will follow up with a reminder to renew before sending out your new card. Our membership form has been updated so you can use your prefered name when you renew, even if different than the name you use for billing purposes.
